Post 18 transition

ISW Care understands the need and importance for young people to have a clear pathway into post 18 services which includes sustainable accommodation, educational and work opportunities.

Every young person will be the focus of our work in achieving safe and secure post 18 support and transition.

In order to do this ISW Care will accurately identify the needs of the young person by initially seeking their wishes and feelings with regard to Post 18 accommodation and services.  Importantly we will work in partnership with a varied range of support services and key stakeholders to set up Post 18 support and accommodation.

ISW care will seek to respect our young person’s rights and basic needs.

We will ensure our young person is:

  • Treated with Equality and respect
  • Safeguarded from danger and harm
  • Supported and cared for specific to their needs, whilst enabling him/her to make informed choices.
  • Ensure they fully understand information in their preferred method of language or communication
  • To advocate on behalf of the young person

When the young person is nearing his/her 18th birthday ISW Care will start the process of completing our in house ‘Post 18 Assessment’. This will aid and assist the pathway plan for the young person for post 18 services. From this ISW care will then seek to hold a ‘Post 18 review meeting’ which will run alongside statutory ‘panel’ meetings between the Local Authority and housing services in our area. The ‘Post 18 review meeting’ will aim to bring together various stake holders in the young person’s life so that there are clear roles and responsibilities to ensure the young person has a seamless transition into post-18 services.
